(b)落实政府采购政策需满足的资格要求：本项目（□是■不是）专门面向中小企业采购，评审时小型和微型企业享受10%的价格折扣。残疾人福利性单位，并将其视同小微型企业。

(b)Qualification requirements to be met to implement government procurement policies: This project is not specifically targeted at procurement by small and medium-sized enterprises. During the evaluation process, small and micro enterprises will enjoy a 10% price discount. Disabled welfare units will be treated as small and micro enterprises.

(c)本项目的特定资格要求：1) 符合《中华人民共和国政府采购法》第二十二条的规定；2) 未被“信用中国”（www.creditchina.gov.cn）、中国政府采购网（www.ccgp.gov.cn）列入失信被执行人、重大税收违法案件当事人名单、政府采购严重违法失信行为记录名单；3) 须具有卫生行政部门核发的《医疗机构执业许可证》；
4) 本项目不允许转包。

(c)Specific qualification requirements for this program: 1) Comply with the provisions of Article 22 of the Government Procurement Law of the People's Republic of China; 2) Not be included in the list of dishonesty and dishonesty by the "Credit China" (www.creditchina.gov.cn), the Chinese government procurement network (www.****.gov.cn), the list of major tax violation cases, and the list of serious illegal and dishonest behavior records in government procurement; 3) Must have a "Medical Institution Practice License" issued by the health administrative department; 4) Subcontracting is not allowed in this project.
